Durable Medical Equipment Prescription Submission Instructions

To facilitate efficient service, please email the required documentation to vaguilar@allpromedny.com. Once we receive your email, a member of our team will call you to confirm receipt of the prescription and to begin processing your request. Kindly ensure that all information is complete, as missing details may result in delays. Please be advised that all items must be processed through your insurance provider prior to dispensing. Coverage is not guaranteed until insurance approval has been obtained.
Include the following items in your email:
- A picture of valid identification (e.g., driver's license)
- Pictures of the front and back of your insurance card(s)
- A picture of the prescription
- Clinical notes from your office visit (if available; if not, we will obtain these from your prescribing doctor)

In the body of the email, kindly include the patient's:
- First and Last Name
- Date of Birth (DOB)
- Address
- Phone Number
- Height & Weight
- Emergency Contact (Name & Phone Number)
